Job description: Swank Center for Memory Care and Geriatric Consultation is looking to hire a Community Educator.Swank Center for Memory Care and Geriatric Consultation is located at 501 West 14th Street, Wilmington, DE 19801. The Swank Center for Memory Care and Geriatric Consultation also has locations in Smyrna and Rehoboth, Delaware.Swank Center for Memory Care and Geriatric Consultation has been selected as a GUIDE model by the Centers for Medicare & Medicaid Services (CMS), enhancing its ability to provide comprehensive dementia treatment plans and provide necessary resources to caregivers.As the Community Educator, you will utilize the GUIDE Model, a pilot program by the Centers for Medicare/Medicaid Services, to enhance access to services, provide health education, and assist community members in navigating healthcare and social services systems for dementia patients and their caregivers.Starting July 2025, the Centers for Medicare & Medicaid Services (CMS) will enroll patients in the ChristianaCare Guiding an Improved Dementia Experience (GUIDE) Memory Program, providing ongoing dementia care services and caregiver support. This nationwide initiative aims to enhance the quality of life for dementia patients, help them stay at home longer, and reduce the burden on unpaid caregivers, often family members. The program includes expert clinicians, the latest Alzheimer's treatment, and a multidisciplinary team of board-certified physicians, social workers, nurse practitioners, and educators. Services include comprehensive assessments, personalized care plans, medication management, monitoring and support, care coordination across healthcare services, caregiver education, training, and connection to community and peer support groups.Work Hours:
  • Monday through Friday from 8am to 4:30pm
  • Participate in an after-hours on-call rotation to provide 24/7 support to patients enrolled in the GUIDE program * Travel to patient homes and other clinical locations is required
  • Must have a valid driver’s license and a registered vehicle for travel
  • Offers opportunity for a hybrid blend of in-person and remote work, ad hoc per practice needs
Job Duties / Responsibilities:As the Community Educator, you will be the primary point of contact for patients, providing personalized support and guidance throughout their care journey. You will collaborate with healthcare providers, social workers, and other team members to develop and implement individualized care plans. Assist patients in accessing healthcare services, community resources, and support programs to address their medical, social, and emotional needs. Educates patients and families about their health conditions, treatment options, and available resources, advocating for patient needs within the healthcare system and community organizations. Regular follow-ups are conducted to monitor progress and adjust care plans as needed. Accurate records of patient interactions, care plans, and outcomes are maintained in the electronic health record system. Quality improvement initiatives are undertaken to enhance the program's effectiveness and efficiency. Training and development are provided to address common issues in neurodegenerative diseases, aging, caregiving, advance care planning, and medication management. Direct support is provided via telephone, video, or in-person, and communication is facilitated. Coordinates with respite and 24-hour caregiver support line staff to assess and address patient and caregiver needs.Education / Requirements:
  • High school diploma required; bachelor’s degree preferred.
  • One-year community and patient experience required, more preferred.
  • Public speaking, group facilitation and excellent writing skills preferred.
  • Computer experience required.
  • Demonstrated ability to work with diverse populations required.
  • Bi-lingual language skills are a plus.
  • Must complete CMS approved GUIDE care navigator training course, this course will be completed after hire.
